I hereby agree to abide by all rules set by A.H.O.S.T. for LOG CABIN DAYS and to participate in a careful, considerate and sober manner. I am providing HAND-MADE/HOMEGROWN ITEMS and am obligated to stay set-up both days of the show.  I also agree to hold LOG CABIN DAYS and A.H.O.S.T.  harmless of loss or damages in which I incur or cause by reason of injury to any person as a result of negligence or willful acts by myself, my agents or my employees in the performance of this contract.

 Log Cabin Days and AHOST will contact the local health department, but you will be responsible for set up in time for and payment of health certificates.

Are you self contained, providing your own power source?

 Yes, we want to participate in Log Cabin Days and have enclosed our check for $100 ($125 if you need electricity).  Our cancelled check will serve as our receipt.

LOG CABIN DAYS ARTS & CRAFTS SHOW

The show is outdoors on a grassy field and will be held rain or shine.  The spaces are 10’ x 20’ and the cost is a non-refundable, flat fee per space (includes both days).  Participants are expected to exhibit both days of the show.  Booths may not breakdown until 5:00 pm on Sunday, May 18, 2008.
 

You will provide your own tent, tables and display.  Any insurance will be provided by the vendor.  Vehicles must be parked in designated areas.
 

Provisions are taken for security.
 

Set up will begin Friday afternoon and must be complete by 8:00 am Saturday morning  of the show.

Sites with electricity available are limited.  Please let us know if you NEED ELECTRICITY as early as possible.  You must provide  your own power cords.  Three pronged heavy duty plugs are required.  There is an additional $10 charge for sites with electricity.
 

All products must be hand crafted and hand made by the artist/crafter.  Imported goods and flea market items will not be accepted.  Vendors are responsible for collecting all applicable sales tax.
 

Pets are permitted on a leash.  Please dispose of all waste appropriately.
 

We look forward to working with you this spring.  Please complete the attached application and return it by April 30, 2010.  Booth assignments will be made by order of applications as they are received.
